Family Support Services

Becoming a parent is an exciting and incredible experience, but new families may need support throughout their journey. All families need a community to support them, and Jefferson County Public Health is here to serve our parents and children with encouragement and resources.
 

Babies First!

Public Health provides weekly/monthly visits to high-risk pregnant women and families with babies/young children who have medical or social risk factors. Families receive health education, case management, care coordination, and referrals. Pregnant women and children up to age 5 are eligible, and priority is given to children on the Oregon Health Plan.

CaCoon

Public Health nurses provide monthly/quarterly care coordination visits for children who are at risk or diagnosed with chronic health conditions or disabilities, and provide in-home assessment, developmental screenings, health education, case management, and referrals. Children with special needs are eligible for CaCoon through the age of 21.

Family Connects-Oregon

Family Connects Oregon (FCO) is a free, nurse home visiting program for all families with newborns. Its mission is to connect families with resources that nurture the whole family and support the child, planting seeds for the future.

Healthy Families Oregon / Healthy Families of the High Desert

Healthy Families Oregon (HFO) is a free family support and parent education home visiting program. This includes screening, referrals, parent coaching and guidance with focus on stress reduction, goal setting, infant mental health and child abuse prevention. Mothers can be enrolled prenatally or before child is 90 days old, and families age out when the child turns 3.

Perinatal Care Coordination

Regional Care Coordination model to help any pregnant women gain access, education and enrollment in programs such as, OHP, WIC, Home Visiting Services, Dental, Behavioral Health, Transportation, Primary Care, Child Birth Classes and Breastfeeding Support. All pregnant or postpartum women qualify for this program.
