The Jefferson County Clerk's office is a passport acceptance agency.  Passport applications are available at the County Clerk's office or online at the U.S. Department of State's website (make sure to follow the printing instructions to avoid rejection of application).  

Passport applications are accepted by appointment only.  Each appointment is for a maximum of 2 applicants.  Please book multiple appointments for additional family members needing application acceptance.

If you arrive late or have not completed the application, we may find it necessary to reschedule your appointment to another date and time.

Please bring the following items with you at the time of your appointment:

  1. Completed passport application (form DS-11). Please DO NOT sign the application prior to coming into our office.
  2. (1) 2"x2", color, passport photo, taken within the past 6 months.  We do not take passport photos at the Clerk's Office.
  3. Proof of identity - valid in-state driver's license or other valid, current photo ID if you do not have a driver's license (children under 18 can use the parent's ID).  If the name on your driver's license (or other photo ID) is different from the name on the application (DS-11), you must submit an original or certified copy of the marriage document, divorce decree or court decree that states the name written on the application. Other valid, current photo ID may required additional forms of ID.
  4. Proof of U.S. Citizenship - original birth certificate  OR Naturalization Certificate if you are a naturalized U.S. Citizen.  The applicable certificate will be submitted to the Passport Agency with your application.  Original certificate is returned to applicant.  (Please note - hospital certificates or records are not acceptable.)
  5. Application Fee AND Execution Fee - A check or money order payable to the U.S. Department of State for the applicable passport fee.  The execution fee of $35 is payable to the Jefferson County Clerk and may be a check or money order.
